Change update: Added the Physical Screening Test Instruction 1220-410. Removed the Physical Screening Test Instruction 1220-100. SF-88 no longer needed. After BUD/S the1306/92 is required. Applicants fill out the 2807-1. Dive Medical Officer fills out the 2808.
