  • PRK/LASIK Form 40-1-4 for MEPS

    Hey all, I thought I would just toss this up here, it's MEPS form 40-1-4, and you need it for getting the LASIK/PRK waiver. I went through this loop recently for like 3 weeks, and THEN they told me this is the form I need, on top of everything I gave before (which was even more detailed information, but MEPS will do what MEPS does). https://drive.google.com/file/d/1UlQ...ew?usp=sharing is the link.

    The MEPS website is about as helpful as MEPS itself, and they're downloads weren't working, so this should help people who need the form.

      Which supporting documents did you submit to MEPS? I have my DD 40-1-4 filled out by my doctor, but I'm not sure what else I need.

        I had to bring my Pre-op notes, Post-op notes, notes from all follow-up visits over 6 months, a letter of clearance signed by the doctor himself, and an "Operative Report" on top of the Form 40-1-4. The Operative report is the "play-by-play" of the surgery, the details of the exact steps taken throughout the procedure. Not sure why they wanted THAT, but it's MEPS, so I provided it.
